Please download and print out the appropriate forms packet below and bring it with you to the Superintendents Office. Please fill out as much information as possible before your scheduled appointment. Generally, substitutes and coaches do not need appointment, but need all forms in packet completed along with two forms of ID. Fingerprinting and CORI must be completed prior to employment. If any forms are incomplete it may delay processing of your payroll.

Massachusetts law now requires all public schools’ employees to have a one-time national criminal background fingerprint check. Unlike the CORI check, this check will examine criminal activity in all states, including Massachusetts.
You are required to submit your fingerprints for the national criminal background check. The fingerprint process requires two steps. The first step is to register online for an appointment to have your fingerprints taken. The second step requires you to go to a location to physically have your fingerprints taken.

The fee is $55 for staff who hold a DESE license and $35 without a DESE license. You may pay with a credit card while registering online, or by check when arriving for your appointment.
NOTE: If you have previously obtained fingerprinting by another school district (within the past 7 years), please contact your previous district and ask them to complete this form by visiting on this link for your determination of suitability. This form should be forwarded to Margaret Cooley at mcooley@awrsd.org or faxed to 978-827-5969.
